A. Except as provided in subsection (B), an applicant shall submit to the Department the following nonrefundable fees for: 1. An initial application or renewal application for certification as a laser technician, $30; and2. A revised/duplicate certificate, $10.B. An applicant for initial certification as a laser technician is not required to submit the applicable fee in subsection (A)(1) if the applicant, as part of the application packet in R9-16-703, submits an attestation that the applicant meets the criteria for waiver of licensing fees in A.R.S. § 41-1080.01.Ariz.
