Section R9-16-104 - AdministrationA. A midwife may submit a written request for the Department to:1. Add the midwife's name, address, and telephone number to a list of licensed midwives on the Department's website; or2. Remove the midwife's name, address, and telephone number from a list of licensed midwives on the Department's website.B. A midwife shall: 1. Notify the Department in a format provided by the Department within five working days after: a. A client has died while under the midwife's care,b. A stillborn child has been delivered by the midwife, orc. A newborn delivered by the midwife has died within the first six weeks after birth; and2. Provide a summary of the: a. Circumstances leading up to the event, andb. Actions taken by the midwife in response to the event.C. A midwife shall: 1. Maintain documentation of: a. Completion of current training in: i. Adult basic cardiopulmonary resuscitation that meets the requirements in R9-16-102(A)(4)(a), andii. Neonatal resuscitation that meets the requirements in R9-16-102(A)(4)(b);b. Except as provided in R9-16-103(A)(2)(b), current certification as a Certified Professional Midwife by the North American Registry of Midwives; andc. The continuing education required in subsection R9-16-105 for at least the previous three years; and2. Provide a copy of documentation required in subsection (C)(1) to the Department within two working days after the Department's request.Ariz. Admin. Code § R9-16-104
