Section R3-2-607 - Entry Permit NumberA. An entry permit number for interstate movement may be obtained from the Office of the State Veterinarian, by calling (602) 542-4293 during the hours of 8 a.m. to 5 p.m. Monday through Friday, excluding state holidays. Any person applying for an entry permit number shall provide the following information: 1. The name and address of the Consignor and Consignee;2. The number and kind of animals;3. The physical address of the origin of shipment;4. The physical address of the shipment's final destination;5. The method of transportation; and6. Any other information required by the State Veterinarian.B. An entry permit number is valid for a maximum of 30 calendar days from the date of issuance unless otherwise indicated on the CVI.C. An entry permit number shall be issued if the animals listed on the Certificate of Veterinary Inspection are in compliance with this Article. To cope with changing disease conditions, the State Veterinarian may refuse to issue an entry permit number or may require additional conditions not specifically established in this Article if necessary to protect animal health in Arizona.D. The entry permit number issued shall be affixed or written on the Certificate of Veterinary Inspection, brand inspection certificate, and any other official documents as follows: "Arizona Permit No. _______" followed by the serialized number.E. The State Veterinarian shall refuse to grant an entry permit number to any person who repeatedly commits the following: 1. Giving false information concerning an entry permit number for transportation of animals,2. Failing to fulfill the conditions of an entry permit number, or3.
