Section 8 AAC 45.025 - Forms(a) The division may require the use of forms for the reporting of any information required by this chapter or by AS 23.30.(b) A person may obtain a list of prescribed and required forms by contacting the division at its offices in Juneau, Anchorage, or Fairbanks.(c) A list or form referred to in this chapter may be obtained by writing to the State of Alaska, Workers' Compensation Division, P.O.
