Section 7 AAC 125.028 - Reporting changes
(a) Not later than 15 days after the recipient knows of the change, the recipient shall report to the recipient's personal care services agency by telephone, by facsimile transmission, by electronic mail, in writing, or in person,
(1) a change in the recipient's
(A) place of residence or living arrangement;
(B) personal contact information;
(C) representative;
(D) medical provider; or
(E) marital status;
(2) a change to the recipient's eligibility to receive home and community-based waiver services under 7 AAC 130, including a change to the recipient's support plan under 7 AAC 130.217 that may duplicate personal care services;
(3) a change to the recipient's other services if those services may duplicate personal care services;
(4) an improvement or decline in the recipient's physical condition; or
(5) a change in household composition or in the number of other recipients receiving services in the recipient's residence.
(b) Not later than 15 days after the personal care services agency learns of the change, a personal care services agency shall report, in a format provided by the department, any change that could affect the recipient's service level authorization or the level of services provided to the recipient. The personal care services agency may submit the report by facsimile transmission, by electronic mail, or in writing.
