Section 7 AAC 115.120 - Occupational therapy evaluation and treatment plan
(a) Before initiating treatment, the occupational therapist shall conduct an initial evaluation of the recipient that includes an assessment of the
(1) recipient's significant past medical history;
(2) diagnosis and prognosis, if established, and the extent to which the recipient is aware of the diagnosis and prognosis;
(3) prescribing health care practitioner orders, if any;
(4) rehabilitation goals and potential for achievement;
(5) contraindications, if any;
(6) summary of any known prior treatment; and
(7) goals of any maintenance occupational therapy as described in 7 AAC 115.110(d).
(b) After conducting the initial evaluation of a recipient, the occupational therapist must establish a written treatment plan. The plan must specify the diagnosis, the anticipated treatment goals, and the type, amount, frequency, and duration of each service. No more than 14 days after the plan is developed or changes are made to service levels, the treatment plan must be signed by a physician, advanced practice registered nurse (APRN), or physician assistant (PA) enrolled under 7 AAC 105 - 7 AAC 160, The department will not pay for services provided more than 14 days after the treatment plan is developed or changes are made to service levels if the treatment plan has not been signed.
(c) After the treatment plan is signed as required under (b) of this section, a physician, APRN, or PA enrolled under 7 AAC 105 - 7 AAC 160 shall review and sign the treatment plan as often as the recipient's medical condition requires or if changes are made to the treatment plan, and no less often than
(1) every six months for recipients under three years of age;
(2) annually for recipients three years of age or older and under 21 years of age;
(3) every 30 days for recipients 21 years of age or older.
(d) The occupational therapist shall record in the recipient's clinical record any changes made to the treatment plan.
