Section 7 AAC 56.650 - Adoptive and guardianship home application and notification regarding application
(a) An agency shall provide an application form for prospective adoptive or guardianship applicants. The application form must include
(1) the name and birth date of each applicant and the name and age of each member of the individual's household;
(2) the phone number and electronic mail address, if any, of the applicant;
(3) the race and tribal affiliation of the applicant, if applicable;
(4) the physical location and mailing address of the applicant's residence;
(5) names, phone numbers, and addresses of four references for the applicant;
(6) a release for the applicant and for each adult living in the applicant's home, authorizing the agency to review criminal justice information, medical records, and licensing records that are relevant to the person who is the subject of the release and to request a protective service records check on the subject of the release;
(7) two sets of fingerprints of each person required to provide a release under (7) of this subsection in order for the department to submit the fingerprints to the Department of Public Safety for the purpose of conducting state and national criminal background checks from criminal justice information received under AS 12.62 and regulations adopted under AS 12.62;
(8) whether the application is for adoption or guardianship;
(9) evidence that the applicant has completed orientation or training required by the agency, if applicable; and
(10) other information in order to enable the agency to determine if the applicant meets the requirements of 7 AAC 56.660; in this paragraph, "other information" includes education, training, marital history, religious preference, children, occupation, interests, annual income for the previous year, financial resources, debts, expenses, employment history, and any insurance coverage required by the agency.
(b) The agency in response to an application shall conduct a home study under 7 AAC 56.660 to assess the applicant's appropriateness to parent an adoptive or guardianship child.
(c) The agency shall notify an applicant in writing within 30 days of completing the home study of the acceptance or denial of the applicant's application.
(d) If an applicant is not accepted, the agency shall provide the applicant with the reasons why a child will not be placed in the applicant's home by the agency.
