Section 7 AAC 56.540 - Discharge(a) An agency shall make diligent efforts to involve the following individuals in planning the discharge of a child: (3) child placing workers in the agency;(4) foster parents or residential staff responsible for the child's care;(5) the state placing worker, if applicable.(b) At discharge, the agency shall include the following in a child's record: (1) a discharge summary showing services provided during care, the growth and accomplishments, assessment of needs that remain to be met, and recommendations about the services needed to meet these needs;(2) date of discharge, reason for discharge, and the name and relationship of each person or agency to whom the child was discharged;(3) aftercare recommendations.Eff. 1/1/2001, Register 156Authority:AS 44.29.020
