Section 7 AAC 47.555 - Application

(a) An individual may apply for assistance under AS 47.45.301-47.45.309 and 7 AAC 47.545 - 7 AAC 47.599 by

(1) submitting an application;

(2) certifying on a form provided by the department to the accuracy and truthfulness of information provided under (1) of this subsection; and

(3) providing documentation requested by the department to verify information provided under (1) of this subsection.

(b) Application may be made by the individual seeking assistance or by a responsible person acting on the individual's behalf.

(c) If the individual discontinues assistance under AS 47.45.301 - 47.45.309 and 7 AAC 47.547 - 7 AAC 47.599, the individual may reapply by submitting the forms specified in (a) of this section.
