Section 7 AAC 34.990 - DefinitionsIn this chapter, unless the context requires otherwise,
(1) "certified copy" means a copy of the original document that has been certified to be a true and correct copy of the original (A) by the person who signed the original document;(B) by a notary public or other person authorized to administer oaths under AS 09.63.010; or(C) if a notary public or other person authorized to administer oaths under AS 09.63.010 is unavailable, in accordance with the provisions of AS 09.63.020;(2) "debilitating medical condition" has the meaning given in AS 17.37.070;(3) "department" means the Department of Health and Social Services;(4) "minor" means a patient under the age of 18 who has not had the disabilities of minority removed under AS 09.55.590;(5) "parent" means a biological or adoptive parent who retains the legal rights to make medical decisions regarding the care of the minor;(6) "patient" has the meaning given in AS 17.37.070;(7) "physician" has the meaning given in AS 17.37.070;(8) "primary care-giver" has the meaning given in AS 17.37.070;(9) "qualified debilitating medical condition" has the meaning given "debilitating medical condition" in AS 17.37.070;(10) "qualified physician" has the meaning given "physician" in AS 17.37.070;(11) "registry identification card" has the meaning given in AS 17.37.070;(12) "written documentation" has the meaning given in AS 17.37.070.Eff. 6/1/99, Register 150Authority:AS 17.37.010
