Section 7 AAC 34.090 - Updating information(a) In accordance with AS 17.37.010(g), a patient to whom a registry identification card has been issued must notify the department of a change in the patient's name, address, physician, or primary care-giver within 10 days of the change.(b) In accordance with AS 17.37.010(h), a patient to whom a registry identification card has been issued shall return the card to the department within 24 hours of being diagnosed by the patient's physician that the patient no longer has a qualifying debilitating medical condition.Eff. 6/1/99, Register 150The department's address to update information under 7 AAC 34.090(a) or to return a registry identification card under 7 AAC 34.090(b) is Marijuana Registry, Department of Health and Social Services, P.O.
