Section 7 AAC 26.845 - Application for license(a) An applicant for a license as a paramedic shah apply in writing on a form provided by the department. (b) The application will be provided by the department and must contain the applicant's name, age, mailing address, and geographical address if different from the mailing address, and any other information that the department considers necessary.
