Section 7 AAC 12.327 - Interdisciplinary team
(a) A full-service hospice agency shall designate an interdisciplinary team composed of individuals who provide or supervise the care and services offered by the agency. The interdisciplinary team must include at least a
(1) physician;
(2) registered nurse;
(3) social worker described in 7 AAC 12.335 and 7 AAC 12.700;
(4) pastoral or other spiritual counselor; and
(5) volunteer coordinator or representative, if applicable.
(b) If the agency provides one or more of the following services, the interdisciplinary team may also include a representative for that service:
(1) physical therapy;
(2) occupational therapy;
(3) speech pathology;
(4) home health services.
(c) Nothing in this section precludes an agency from designating additional individuals to join the interdisciplinary team on a regular or as-needed basis.
(d) The interdisciplinary team shall
(1) participate in the development and maintenance of the plan of care for each client of the agency;
(2) establish the policies governing the daily provision of hospice care and service;
(3) schedule and attend interdisciplinary team conferences at a frequency that is based on client needs;
(4) document conference findings and conclusions; and
(5) review and make any necessary revision of each plan of care as required under 7 AAC 12.319.
(e) Each member of the interdisciplinary team must be qualified, in terms of applicable education and training. If the individual's profession requires licensing or certification, that license or certification must be current. The agency must maintain a written record for each team member that includes verification of applicable education, training, license, and certification. The agency shall review these records annually to ensure the records are current and accurate.
