Acceptable documentary evidence shall be prescribed by the State Registrar, following generally the national standards recommended by the agencies responsible for national vital statistics, and for the use of records in the interest of national security. In general such documents must be records established in the past, preferably near the time of birth, but at least five years before such application; they must contain specific reference to at least one of the facts to be proved: date of birth, place of birth, parentage; they must be identified as pertaining to the person in question. Such documents usually are records made by various agencies, institutions, or persons for some other purpose but showing incidentally as identifying items one or more of the facts to be proved; or they may be an actual recording of the facts of birth. One affidavit of personal knowledge by a competent person with adequate knowledge of the facts, if found acceptable by the State Registrar, may be used in lieu of one of the three required documents; such affidavit need not be five years old.
