Section 4 AAC 18.005 - Application for employment in certificated positionA district shall include on the district's application for employment for all positions requiring a certificate under 4 AAC 12
(1) an inquiry into whether the applicant has ever been employed in the public schools of this state in a position requiring a teacher or an administrator certificate issued by the department;(2) if the applicant has been employed in a certificated position in the state, a requirement that the applicant provide the dates, places, and job titles for the employment; and(3) a statement that failure to provide truthful and complete information on this application would be a violation of the code of ethics adopted by the Professional Teaching Practices Commission under 20 AAC 10.020.Eff. 9/2/2011, Register 199
