Section 20 AAC 05.570 - Notification of damage, loss, or theft
(a) The holder of a permit, a vessel permit, or a vessel license shall promptly notify the commission of the damage, loss, or theft of a permit card, certificate, vessel permit, or vessel license. The holder of a permit or vessel license may request a duplicate by submitting to the commission a Request for Duplicate form provided by the commission, along with the appropriate fee under 20 AAC 05.1910 for replacement of the designated item. On the affidavit portion of the form, the applicant shall set out, under penalty of perjury, the facts describing when, where, and how the damage, loss, or theft occurred. Submission of a Request for Duplicate form to the commission causes the original permit card, certificate, vessel permit, or vessel license to be invalidated for the remainder of the calendar year.
(b) A carbon copy of the Request for Duplicate form may be used for fishing only if the form has been submitted to the commission through a commission agent who has certified the form as complete by signing and validating it. The carbon copy may be used for fishing for no more than 14 days unless extended by the commission. The agent will not certify the form as complete unless the form is accompanied by the appropriate fee under 20 AAC 05.1910 for replacement of each designated item and an envelope, addressed to the commission, with adequate postage.
(c) An otherwise properly completed Request for Duplicate form that is not signed and dated by a commission agent as described in (b) of this section must be submitted to the commission and must be accompanied by the appropriate fee under 20 AAC 05.1910 for replacement of each designated item. A copy of the form cannot be retained for fishing.
(d) For the purposes of receiving and certifying as complete a Request for Duplicate form, an employee of the Department of Fish and Game or of the Department of Public Safety, division of fish and wildlife protection, is an agent of the commission.
(e) Repealed 12/31/88.
(f) During the period June 1 - July 31 each year, the following procedures supersede (b) of this section for the Bristol Bay administrative area, and govern requests for duplicate salmon entry or interim-use permit cards:
(1) A request for a duplicate Bristol Bay salmon permit card cannot be signed and certified as complete by a commission agent unless the agent has obtained approval from a commission employee either telephonically or in person.
(2) If issuance of the duplicate card is approved, the permit holder will be issued a duplicate permit card or a signed Request for Duplicate form validated for a time certain, not to exceed 14 days.
(3) If the request for a duplicate card is denied, the permit holder will be given a signed copy of the form indicating the reason and date of the denial and will be advised of his or her right to request a hearing as provided by 20 AAC 05.1800 - 20 AAC 05.1870. The copy of the form may not be used for fishing.

By its own terms, 20 AAC 05.570(e) was repealed 12/13/86. That subsection was revived, with a new self-contained repeal date, by the Commercial Fisheries Entry Commission's adoption, effective 11/22/87, Register 104.
