Section 18 AAC 74.420 - Certification requirements for operators of small, untreated or small, treated water systems
(a) An individual seeking certification under 18 AAC 74.400 - 18 AAC 74.440 must submit a completed application to the department, for the level of certification sought
(1) on a form provided by the department;
(2) accompanied by the fee required by 18 AAC 74.870.
(b) Except as provided in (d) - (g) of this section, the department will certify an applicant who meets the
(1) examination requirements of 18 AAC 74.430;
(2) experience and education requirements of 18 AAC 74.440; and
(3) fee requirements of 18 AAC 74.870.
(c) An applicant must disclose past or pending disciplinary action by another certifying agency.
(d) When considering an individual's application for certification, the department may consider past or pending disciplinary action that another certifying agency imposed on the applicant in the applicant's capacity as an operator.
(e) The department may deny certification to an applicant whose certification has been revoked by another certifying agency.
(f) The department may deny certification to an applicant determined to have falsified or with held information on the applicant's application.
(g) when acting under (d)-(f) of this section, the department will consider the factors set out in 18 AAC 74.830(b).
