Section 12 AAC 02.900 - Name and address changes
(a) A person licensed, registered, or certified by a board or commission listed in AS 08.01.010, or in an occupation listed in AS 08.01.010, shall maintain a current, valid mailing address on file with the division. The division will use the latest mailing address on file for official communications, notifications, and service of legal process. If an applicant or licensee has opted into electronic communication, the electronic mail address provided may be used for official correspondence and notifications.
(b) A licensee shall notify the division of a change to the licensee's physical address, mailing address, or electronic mail address on file with the division. The licensee may submit notice in writing or through the MyAlaska professional licensing portal on the department's Internet website.
(c) If a licensee has a change of name, the licensee shall submit to the division within 30 days of the change of name
(1) notification of the change of the licensee's name, on a form provided by the division that has been completed by the licensee and notarized;
(2) a copy of the marriage certificate, court document, or other legal document verifying the change of name; and
(3) the fee established in 12 AAC 02.105 for a name change.
(d) The division will issue a new license showing the change of name if a licensee meets the requirements in (c) of this section.
