License applications, annual license renewal applications, and all other applications or submissions required by these rules shall be mailed, faxed or emailed to the Executive Director. All license applications, annual license renewal applications and any other application or submission will be deemed filed when received by the Executive Director. The Executive Director may reject an application that is inaccurate, incomplete, or for which all fees due have not been paid at the time the application is filed.
