Section 480-6-1-.01 - Multiple Worksite Report

(1) The Labor Market Information (LMI) Division is responsible for, among other duties, collecting information for statistical purposes. The LMI Division uses, among other methods, certain forms to collect such information. Much of the information collected is shared or forwarded to the Federal Bureau of Labor Statistics (BLS) as required by federal law.

(2) The "Multiple Worksite Report" form has been approved, O.M.B. No. 1220-0134, and is authorized by federal law 29 U.S.C. 2.

(3) The information requested on said "Multiple Worksite Report" is hereby mandatory and required.

(4) The BLS will use the information collected on said "Multiple Worksite Report" for exclusively statistical purposes and may share it with other federal agencies of exclusively statistical purposes. BLS will hold the information in confidence to the full extent permitted by federal law.

(5) The State of Alabama, Department of Industrial Relations, LMI Division will use the information for unemployment insurance program purposes, statistical purposes, and other purposes as permitted by state law.
