Section 330-X-10-.03 - Reissuance Of Certificates

A licensee requesting a duplicate certificate of licensure under the provisions of Code of Ala. 1975 Section 34-11-12, shall submit a request in the format specified by the Board. Upon receipt of the request accompanied by the required fee a duplicate certificate will be issued
