Incident: Hurricane Matthew.
Incident Period: 10/03/2016 through 10/19/2016.
Effective Date: 10/25/2016.
Physical Loan Application Deadline Date: 12/16/2016.
EIDL Loan Application Deadline Date: 07/17/2017.

Primary Counties: (Physical Damage and Economic Injury Loans): Nassau, Seminole.
Contiguous Counties: (Economic Injury Loans Only): Georgia, Camden, Charlton.
All other information in the original declaration remains unchanged.
