SUPPLEMENTARY INFORMATION:

Title: Pension Claim Questionnaire for Farm Income, VA Form 21-4165.

OMB Control Number: 2900-0095.

Type of Review: Extension of a currently approved collection.

Abstract: VA Form 21-4165 is used to gather information necessary to determine a claimant's countable annual income and available assets due to farm operations. Eligibility to income-based benefits cannot be determined without complete information about a claimant's income. Farm income is not necessarily received on a weekly or monthly basis, and farm operating expenses must be considered in determining income and total assets. If eligibility exists, the information will be used to determine the proper rate of benefits.

An agency may not conduct or sponsor, and a person is not required to respond to a collection of information unless it displays a currently valid OMB control number. The Federal Register notice with a 60-day comment period soliciting comments on this collection of information was published on June 15, 2004, at page 33466.

Frequency of Response: Annually.

Estimated Number of Respondents: 2,075.

Dated: October 19, 2004.
