Section 64-94-8 - Persons and Facilities, Required to Report8.1. Health care Providers and Facilities. All health care providers and facilities, as defined in section 2 of this rule, that diagnose Alzheimer's disease or a related disorder or provide treatment or care for an individual who is diagnosed by a physician to have Alzheimer's disease or a related disorder shall provide a report of each case of the disease or condition as required by this rule.
