Section 64-70-4 - Application Procedures4.1. Applications for grants to offset the cost of providing uncompensated primary care services shall be submitted by May 1 of each year, unless another date is announced by the director, and shall be on forms approved by the director. The director may request appropriate documentation or clarification of the application from the applicant.
