A hospital that provides care for a Medicaid member must provide sufficient documentary evidence that ancillary services for which Medicaid is billed are provided in the diagnosis or treatment of the member and that services are authorized by a licensed provider. Medicaid may not provide reimbursement if a hospital does not provide evidence in accordance with this rule
