Section 201.15 - Restrictions on license (b) A license becomes automatically void without notice if any of the following conditions exist: (1) The license term expires unless the term expires due to a departmental delay, a Federal emergency or State disaster emergency. (2) There is a change in ownership and the Department has not given prior approval.(3) There is a change in the name of the facility, and the Department has not given prior approval.
