Section Saf-C 909.04 - Information Required(a) All persons seeking a medical waiver shall complete and submit DSMV 558, "Application for Waiver of Physical Deficiency", (rev. 04/23).(b) By signing and dating the medical waiver form, each driver-applicant, official of the employing motor carrier, and medical examiner shall certify that they are signing the form under the penalties of unsworn falsification pursuant to RSA 641:3.N.H. Admin. Code § Saf-C 909.04
