Section Saf-C 5703.08 - Procedure for A Person to Obtain His or Her Criminal History Record Information(a) A person requesting his or her CHRI shall appear at the central repository or request it in writing by completing and submitting the criminal record release authorization form, DSSP 256 'Criminal History Record Information Request Form', effective July 15, 2016.(b) A person who appears at the central repository shall be required to complete and submit section I of DSSP 256, 'Criminal History Record Information Request Form', effective July 15, 2016.(c) A person who mails a request for the release of his or her CHRI shall complete and submit both sections I and II of DSSP 256 'Criminal Record Information Request Form', effective July 15, 2016.(d) A request by mail shall be addressed to: Department of Safety
