Section Saf-C 5007.01 - Applications(a) An application for a permit for a supervised display of display fireworks by a municipality, an association, amusement park, other organizations, or person shall be in writing on form DSFM 166.(b) Applications for display shall be submitted to the head of the fire department and to the licensing board, or in the absence of a licensing board, to the chief of the police department or governing board in the municipality where the display shall be given, not less than 15 days in advance of the display.(c) The operator of the display shall provide the commissioner with an approved form DSFM 166 at least 3 business days prior to the intended date of the display.N.H.
