Section Saf-C 311.43 - Form RT 133a, Alternative Fuel Dealer Bond

(a) An applicant shall submit the following on form RT 133a:

(1) Applicant's name and address;

(2) Federal employer identification number, if applicable;

(3) Name and address of surety;

(4) Amount of bond;

(5) Date of document; and

(6) Statement of obligation by licensee.

(b) The applicant shall sign the form.

(c) The surety representative shall sign the form.

(d) A witness shall sign the form.
