Section Saf-C 311.41 - Form RT 132b, Application for Alternative Fuel Dealer License

(a) An applicant shall submit the following on form RT 132b:

(1) Applicant's name and address;

(2) Federal employer identification number;

(3) Location of principal office;

(4) Type of business;

(5) Location of records for audit purposes;

(6) Name and address of representative;

(7) Name and address of principal officers;

(8) Other states where the applicant is licensed as a distributor;

(9) Estimated monthly volume in gallons;

(10) Location of New Hampshire distribution facilities; and

(11) Storage information for each distribution facility, consisting of capacity, product code, and whether the facility is owned or leased.

(b) The applicant shall sign the form.

(c) The applicant shall, as soon as practicable, notify the bureau of a change in the information submitted on the form.
