Section Saf-C 311.19 - Form RT 111, Application for Motor Fuel and Petroleum Products Transporter License(a) An applicant shall submit the following on form RT 111: (1) Applicant's name, address and telephone number;(2) Federal employer identification number, if applicable;(3) Location of records for audit;(4) Statement of RSA 260:42 requirements; and(5) Contact name and e-mail address, if available.(b) The applicant shall print his/her title and sign the form.(c) An application for a motor fuel and petroleum products transporter license shall be filed each fiscal year.N.H.
