Section Saf-C 311.16 - Notice of Suspension

(a) The department's notice of suspension shall contain the following:

(1) Licensee's name, address and federal employer identification number, if applicable;

(2) Account number;

(3) Date of suspension;

(4) Statement of action(s); and

(5) Statement of available options.

(b) The commissioner or his/her designee shall sign the form.
