Section Saf-C 311.08 - Forms BDF Schedule of Disbursements 5, 6, 6F, 7, 8 and 9

(a) An applicant shall submit the following on forms BDF schedules 5, 6, 6F, 7, 8 and 9:

(1) Distributor's name, license number and federal employer identification number, if applicable;

(2) Schedule numbers as listed below:

a. BDF schedule #5 - Gallons delivered - taxable;

b. BDF schedule #6 - Gallons delivered to New Hampshire licensed distributors;

c. BDF Schedule #6F - Gallons of dyed diesel and dyed kerosene sold for tax exempt purposes;

d. BDF Schedule #7 - Gallons exported;

e. BDF schedule #8 - Gallons delivered to US Gov't Armed Forces (Bulk) tax exempt; or

f. BDF schedule #9 - Gallons delivered to State/Local Gov't (Bulk) tax exempt;

(3) Product type;

(4) Date, including month and year;

(5) List of shipments, including:

a. Carrier name and federal employer identification number;

b. Mode of transportation;

c. Points of origin and destination, both city and state;

d. Purchaser's name and federal employer identification number;

e. Date delivered;

f. Document number;

g. Gross gallons; and

h. Net gallons; and

(6) Page total.
