"Sworn report" means paragraph V of form DSMV 426 submitted by a law enforcement officer and signed by a justice of the peace, notary public, or commissioner of deeds, before whom the law enforcement officer personally appeared and recognized his or her obligation to tell the truth but does not include a requirement that the law enforcement officer raise his or her right hand and take a formal oath.
