Section Saf-C 2304.04 - Replacement Decal(a) In the event an original decal is lost, stolen or damaged, the owner shall complete Form RDMV 415 "Application for Replacement Boat Decals" (rev. 08/11) and submit it to the department.(b) Upon receipt of a completed form RDMV 415 and valid vessel registration certificate, and payment of the required fee of $3.00, the department shall issue a replacement decal.N.H.
