Section Saf-C 2007.02 - Application for Transporter Registration(a) An applicant for a transporter registration shall complete and submit an application RDMV 725 (rev 12/15) to the department.(b) If the business location is leased, the applicant shall submit a legible copy of the current, validlease agreement.(c) Each person listed on the application as a member, partner, officer, or director of the applicant shall provide a certified copy of his or her criminal record.(d) The applicant shall provide a statement whether the applicant has ever been subject to a civil judgment connected with a motor vehicle business.N.H.
