Section Saf-C 2002.01 - Forms(a) All forms may be obtained in person, by telephone, by mail or electronically, from the Dealer Desk, Division of Motor Vehicles, 23 Hazen Drive, Concord, N.H., 03305, or at any motor vehicle substation.(b) Whenever a person is required to complete a form, the following shall apply: (1) Forms shall be filled out completely;(2) Forms shall be typewritten or completed in a legible manner; and(3) Forms requiring a signature shall require such person to certify, by the individual's signature that the information on the form is true and correct to the best of the individual's knowledge and belief, subject to the penalty for making unsworn false statements under RSA 641:3.N.H.
