Section Saf-C 207.01 - Insurance Certificates

(a) An SR-22 certificate filed pursuant to any law or rule shall include:

(1) The insured's name and date of birth;

(2) The insured's address;

(3) The name of the insurance company;

(4) The policy number;

(5) The effective date of the policy; and

(6) The type of coverage in effect.

(b) An SR-26 certificate filed pursuant to any law or rule shall include:

(1) The insured's name and date of birth;

(2) The insured's address;

(3) The name of the insurance company;

(4) The policy number;

(5) The effective date of the policy; and

(6) The effective date of the cancellation or termination of the policy.
