Section He-P 2153.09 - Change in Name(a) When a certificate holder intends to change the name of its business, the certificate holder shall submit a written request to the department for a new certificate at least 30 days prior to the intended date of name change.(b) The written request shall include:(1) The reason for requesting a new certificate;(2) The name of the business as it appears on the existing certificate;(3) The name of the business as the certificate holder requests it to appear on the new certificate; and(4) The date upon which the change in name is intended to occur.(c) Following receipt of the items required by (b) above, the department shall issue a revised certificate reflecting the change in name. The certificate number and expiration date shall remain the same as it was on the immediately preceding certificate.N.H. Admin. Code § He-P 2153.09
