Rule 15-16-1-52.9.19

The patient record shall contain information on any unusual occurrences, such as the following:

1. treatment complications;

2. accidents or injuries to the patient;

3. morbidity;

4. death of a patient; and

5. procedures that place the patient at risk or that cause unusual pain.
