Rule 15-16-1-83.8.5 - Renewal of License

License shall be renewable annually upon:

1. Filing and approval of an application for renewal by the licensee.

2. Submission of appropriate licensure renewal fee.

Maintaining compliance with requirements as set forth in these minimum standards in regard to physical plant, staff, services, and operation.
