Rule 15-16-1-82.5.2

Before the certification expires, the certified private review agent may renew its certification for an additional two (2) year term, if the certified private review agent:

1. Is otherwise entitled to be certified;

2. Pays to the Director the renewal fee of $1,000.00; and

3. Submits to the Director:

a. A renewal application on the form that the Director requires

b. An update of information as required under Part IV of these rules and regulations

c. An annual report.
