The written policies and procedures shall describe the methods by which continuity of care is assured for the patient. These methods shall include, but not be limited to, providing the facility, program component, or other service provider to which the patient is referred with the following:
1. Background information on the referral;2. Information on the patient's treatment, for example, current treatment, diagnostic assessments, and special requirements;3. Treatment objectives desired;4. Suggestions for continued coordination between the referring and the receiving resource;5. Special clinical management requirements; and6.
