Rule 15-16-1-51.48.1

A discharge summary shall be entered in the patient's record within fourteen (14) days following discharge. The discharge summary shall include but not be limited to:

1. Reason for admission;

2. Brief summary of treatment;

3. Reason for discharge;

4. Assessment of treatment plan goals and objectives; and

5. Recommendations and arrangements for further treatment, including prescribed medications and aftercare.
