Rule 15-16-1-51.45.1

Progress notes shall be recorded by the physician, psychiatric mental health nurse practitioners, nurse, social worker and, when appropriate, others significantly involved in treatment. The frequency of progress notes is determined by the condition of the patient but should be recorded at least monthly.
