Rule 15-16-1-51.43.8

An educational assessment of each patient shall be completed by a qualified special education teacher and entered into the patient's record. The assessment shall include, but not be limited, to the following information:

1. Previous school history with regard to academic, social, and behavioral skills and deficits as well as school disciplinary actions; and

2. Psychometric measures as appropriate for the child/adolescent.
