Care Team shall mean a group of individuals responsible for the development of each patient's care plan. The care team shall consist of, but not be limited to, the physician, podiatrist, nurse practitioners, physician assistants, clinical nurse specialists, and pertinent members of the agency staff, the patient and member of his/her family.
