Parent(s) or legal guardian(s) shall, prior to and upon admission and as needed during the period of service to his/her child, receive a written statement of the services provided by the Pediatric Skilled Nursing Facility including those offered on an "as needed" basis. They shall also receive a statement of related charges including any charges for services not covered under the Pediatric Skilled Nursing Facility's basic per diem rate.
